Family Connects Durham offers a free nurse home visit to families of newborns (up to 12 weeks in age) in Durham County, North Carolina. Please provide the following information and one of our Program Support Specialists will get in touch with you. They will help connect you with one of our nurse home visitors who will confirm visit details with you directly.
Our visits are being conducted both in-home (or some other suitable location that you and your family might find most comfortable) and via telehealth (by phone or a video call). Our nurses continue to practice all relevant safety and health guidelines from local, state, and federal agencies. We can’t wait to see you!
